How to Become a Patient

    Describe Your Concern

    Tell us briefly what's going on with your foot or ankle. Our staff will help determine the right type of appointment and which location works best for you.

    Confirm Your Appointment

    All appointments are confirmed by phone. If you submitted an online request, we'll call you back at the number you provided. Please answer calls from unknown numbers while awaiting our call.

    Arrive & Get Care

    Both offices have free parking and easy access. We strive to accommodate your scheduling needs with morning, afternoon, evening, and Saturday availability.

What to Bring to Your First Visit

  • Photo ID and insurance card(s)
  • List of current medications
  • Any relevant prior X-rays or imaging
  • Shoes you wear most often (we may want to examine them)
  • List of questions or concerns you'd like to discuss

Insurance & Payment

Dr. Forni is a contracted provider with many major insurance carriers including Blue Cross/Blue Shield PPO, United Healthcare, Aetna, Cigna, Humana, and more. Call us to verify your specific plan is accepted. We are happy to discuss options for patients with high deductibles.

Patients of All Ages Welcome

AFP Podiatry provides family-oriented care from newborns through senior adults. Dr. Forni has extensive experience treating pediatric foot conditions and makes children feel comfortable and at ease during their visit.

Urgent & Emergency Care

If you have a non-life-threatening foot or ankle emergency — sprain, fracture, infection, cut, or foreign object — call the office first. Most emergencies can be treated in-office faster and at lower cost than the ER. We have on-site X-ray and ultrasound. For life-threatening emergencies, call 911.

Office Hours & Scheduling

Both offices are open Monday through Saturday. We offer morning, afternoon, evening, and Saturday appointments by arrangement. Scheduling staff available Mon–Sat, 9 AM – 9 PM. Emergency line available 24/7.
